We will continue to update policies, procedures and educate staff per CDC guidelines.  We have dedicated isolation for residents who show symptoms of COVID-19 or those that test positive for COVID-19.  At this time, we have adequate Personal Protective Equipment (PPE).  We will continue to conduct rounds to assure PPE is used properly and we will continue to follow the Executive Order guidelines. We will also continue to provide health checks to all staff members prior to work.  Patients will be screened and monitored for signs and symptoms of illness each shift.  At this time all types of visitation are suspended.  Please continue to monitor our website to up to date information.

While we have taken steps to prepare our staff and community, we
need your help to minimize not only our community but every ones

Visitation Guidelines

On March 13, 2020 federal authorities recommended restricting visitation to prevent

outbreaks of COVID-19 in nursing and assisted living facilities. We appreciate the efforts of

families, residents, staff and our providers responding to the COVID-19 Crisis. Ensuring the

health and safety of residents has been challenging and required tremendous work. Nursing

and assisted living facilities continue to be at risk of experiencing outbreaks of COVID-19

and the residents of our facilities are often in a high-risk category for serious complications

from COVID-19. The continued use of technology to keep families connected as much as

possible will continue. However, to balance the needs of families and residents to see each

other in person with the need to protect residents from COVID-19, this letter outlines the

criteria set by Department of Health and Human Services for allowing  visitation.

Current data indicate that risk of transmission in outdoor settings is lower compared to

indoor settings.

The following is a list of Visitation Guidelines for

Our  Assisted Living Facility:

1. Visits have to be scheduled in advance and visits will be dependent on

availability of suitable space and sufficient staffing and PPE (Personal Protective Equipment)

at the facility to meet resident care needs.

2. At present time, one visit per time slot will be permitted.

3. Shaire will ensure appropriate personal care and supervision is provided for

the safety of the resident by taking into consideration the needs of the

resident and the situation.

4. Shaire will require pre-visit orientation, and screenings of visitors to include

presence of symptoms and known exposure to COVID-19, and ensure visitors bring

and wear a mask for face covering. This orientation and screening will be done 10

minutes prior to the start of your visit.

5. Shaire must screen for fever and other symptoms associated with COVID-19

(fever equal to or greater than 100.degree F, cough, shortness of breath, sore throat,

muscle aches, chills or new onset of loss of taste or smell) prior to resident being

transported to the designated space.

6. Visitors must cooperate with the facility's screening process at each visit and

attest to not having signs or symptoms or current diagnosis of COVID-19; if they have

had COVID-19, they must provide documentation (e.g., doctor's note) that they no

longer meet CDC criteria for transmission-based precautions.

7. Any individuals with symptoms of COVID-19 infection (fever equal to or greater

than 100.0 F, cough, shortness of breath, sore throat, muscle aches, chills or new

onset of loss of taste or smell) will not be permitted to visit with a resident.

8. Any individual that develops within 2 days of visiting a resident a diagnosis of

COVID-19 or signs and symptoms such as fever, cough, shortness of breath, sore

throat, muscles aches, chills, or new onset loss of smell or taste, must immediately

notify the facility of the date they were visiting and

the resident they were in contact with.

9. Visitors must be limited to no more than two individuals at a time per resident.

Children visitors must be at least 12 years old, able to wear a mask covering during

the entire visitation and remain with their guardian who will be responsible for

assuring that all safety measures are followed. A child is counted

as one of the two visitors.

10. Visitors must bring and wear mask covering both the mouth and nose for the

entire visit. No outside food or beverage will be permitted during a visit. Pets are

not permitted during a visit. Shaire will refuse entry to, and may cease visitation by

visitors who do not comply with wearing a mask.

11. Shaire will provide alcohol-based hand rub to visitors and demonstrate how to

use it appropriately if necessary.

12. Visitors, residents and staff must use alcohol-based hand

rub before and after visitation.

13. A Shaire staff member will be available to transport residents to and from the

visitation session, screen the visitors, and remind the visitors of the visitation

protocols and infection prevention measures to be taken during the visit. Staff and

residents must wear a face mask for the duration of the visit.

14. Shaire’s designated area for the resident and for the visitors will be clearly

marked maintaining with 6 feet or more of separation. Visitors must stay in the

designated location and only visit the resident they intend to visit. At the time of

your visit, you will need to be seated prior to your loved one being brought out to

visit and remain seated during the visit. Visitors are to remain seated until the

resident is escorted back into the facility.

15. Visitors must remain at least 6 feet from the resident and staff at all times

during the visit.

16. Shaire has the discretion to alter visitation practices based on disease

transmission in the facility or community, visitor non-compliance, or other factors.

17. Shaire may limit the length of any visit, the days on which visits will be

permitted, the hours during a day when visits will be permitted, and the number of

times during a day or week visitation may occur.

As you consider an in-person visit, please keep some details

in consideration moving forward.

1. Would your loved one be able to hear you effectively being 6 feet apart?

2. Would vision be an issue sitting 6 feet apart?

3. Would your loved one be cognitively able to understand the requirements of

remaining 6 feet apart? Having no physical contact?

Staying seated during the visit?

4. Would visiting upset you or your loved one having to remain 6 feet apart?

Visits will begin Tuesday September 8, 2020. Each visit will consist of a 30 minute schedule

window. The first 10 minutes is for screening followed by 20 minute visit with resident.

Shaire will do our best to make sure all residents have visits that want them. All residents

that want visits will be allowed to have one prior to scheduling a second or third resident

visit. Shaire has the availability to schedule 10 resident visits per visitation day. The

following is a list of Visitation Days and Times available:

Monday, Tuesday, Thursday 9:30am–12:00pm, 1:30pm–4:00pm

Time slots available will be: 9:30am-10:00am, 10:00am-10:30am, 10:30am-11:00am,

11:00am-11:30am, 11:30am-12:00pm, 1:30pm-2:00pm, 2:00pm-2:30pm, 2:30pm-3:00pm,

3:00pm-3:30pm, 3:30pm-4:00pm

*THESE VISITS REQUIRE A 24 HOUR ADVANCE NOTICE*

We ask in addition to these days being used for available visitation days, to please use

these time slots to call ahead for face to face window visits on Saturday and Sundays.

Hospice Services

Shaire Center coordinates with local Hospice Agencies to provide full Hospice Services as well as Palliative Services.

Discharge Planning/Care        Planning

Shaire Center has Registered Nurses who coordinate the care planning process.  An Interdisciplinary team meets periodically to discuss Goals, Interventions as well as Discharge planning.

Conflict Resolution

Our facility has an open door policy and welcomes concerns/compliments.  Issues can be presented in person or in writing.  All issues will be addressed and a resolution will be provided.

John Corder, M.D. is our Medical Director.  Tyler Allen, P.A. is also a provider for our facility.

Dining Programs

Our kitchen boasts a high sanitation grade which speaks volumes of our trained staff and their dedication to cleanliness. Our Registered Dietitian and Dietary Managers plan and prepare three appetizing meals per day, with nutritional snacks being offered to help meet therapeutic dietary needs.
Our Speech Therapists are also part of our interdisciplinary team making recommendations and closely monitoring resident’s progress for proper diet consistencies.

Shaire Technology

Shaire Center provides large flat screen Televisions for each patient.  Cable TV and wifi also provided at no additional cost.   Adaptations provided for visual/hearing deficits.

Self Care Skills

Occupational Therapy teaches functional and self care skills through activities of daily living including feeding, dressing, bathing, and grooming through the use of techniques, which improve motor coordination, perception, and cognitive skills.  Our fully equipped kitchen allows our therapists to integrate treatments and sessions to re establish daily independent living skills for the transition back to home.

Physical Therapy addresses overall function and/or pain management such as the ability to walk again after illness through the use of therapeutic exercise mobility, balance, and gait training exercises.

Speech Therapy

Speech/Language/Swallowing

Speech Therapy is speech/language oral sensory motor training. It addresses communication and swallowing disorders, and difficulties by strengthening muscles, and teaching compensatory techniques to increase communication and proper nutrition. 

 

Our Rehabilitation Department is dedicated to helping residents achieve their maximum capabilities. We strive to enhance resident quality of life by developing and implementing a personalized plan of care that addresses the total needs of each individual.  Our therapists address physical pain and limitations through manual therapy and the use of modalities such as ultra sound, electrical stimulation and thermal agents. Individualized home programs are established which always include specific exercises and stretches to allow patients to continue to progress at home. Within Shaire Center, you'll find compassionate, qualified staff who will greet you with a “Can Do” attitude. This is the day the Lord has made. We will rejoice and be glad in it. This is one my favorite Bible verses. If we stop and let these words sink in, God made this day, ordained this day, designed this day. God isn’t on vacation; He still occupies the universe’s only throne. Each day emerge from God’s drawing room. Whatever the day brings, if it starts out badly we need to change our view and imitate the resolve of Psalmist: “I will rejoice and be glad in it” Lots of times we say we will rejoice when this day is over, but I don’t know about rejoicing in it this day. But God invites us to be happy while we are in the day. Paul rejoiced in prison, David wrote psalms in the wilderness, Jonah prayed in the belly of a whale, and Jesus prayed in the garden. Could we rejoice smack-dab in the midst of our day, imagine the difference if we could? Suppose neck deep in a terrible, horrible, no good, bad day we resolve to give it a chance. We would trust more, stress less, and amplify gratitude. Before long, the day is done and surprisingly turns out decent. So decent, in fact, that we resolve to give the next day the same fighting chance. Giving the day a chance works, each day holds: 84,000 heart beats, 1,440 minutes, a complete rotation of the earth, a circle in the sun dial, both a sunrise and sunset, a brand spanking new unsoiled, untouchable, uncharted and un-used day, a gift of 24 unlived hours. It’s our choice: Rejoice or Not. We no longer have yesterday. It slipped away as we slept. It is gone. We can’t change, alter or improve it. In a hour glass sand won’t flow upward; the second hand of the clock refuses to click backward. We do not yet have tomorrow; we can’t spend tomorrow’s money, celebrate tomorrow’s victories, or solve tomorrow’s problems. We have only today. This is the day the Lord has made. We must be present to experiment it, to enjoy it. Let’s not pile today up with yesterdays regrets or the troubles. We sabotage our day wiring it for disaster, struggling along yesterdays troubles, down loading tomorrow with challenges, and remorse of the past, with anxiety over the future. We aren’t giving the day we’re in a chance ~ Here’s my proposal: First thing in the morning consult Jesus. The Ancient of Days has something to say about our days. He doesn’t use the term day very often in the Scriptures but the few times He doesn’t use the term day very often in the Scriptures but the few times He does use it, it provides a formula for up grading each of our days to blue ribbon status. This is the day the Lord has made we will rejoice and be glad in it. It’s entirely our choice!

Each Day”

 

Each day has 1,440 minutes. Do we value our relationship with God enough to spend a few of those minutes with Him? He deserves at least that much of our time and more. The question is He receiving it from us? Hopefully so, but if we find that we are simply “too busy” for a daily chat with our Father in Heaven, it’s time to take a long hard look at our priorities and our plans for the day ahead. Here’s a tip, organize your life around this simple principle, “God first”. If we place our creator where He belongs, at the very center of our day and our life, the rest of our priorities will fall into place. Each new day is a gift from God and if we are wise we will spend a quiet moment each morning thanking the Giver, our Father God.

Daily life is woven together each morning with the threads of habit and no habit is more important to our spiritual health than the discipline of daily prayer and devotion to God. When we begin each day with our heads lowered and our hearts lifted we are reminded of God’s love, His protection and His commandments. We can align our priorities for the coming day with God’s help, if we have acquired the unfortunate habit of trying to “squeeze” God into the corners of our life. It’s time to reshuffle the items on our do list by placing God first. God wants our undivided attention not the leftover of our day. If we haven’t already done so we need to form the habit of spending quality time with our Father. He deserves it and we all need it.

    Gap Standers    

 Ezekiel 22:30 I looked for a man among them who would build the wall and stand before me in the gap on behalf of the land, so I would not have to destroy it, but I found none.

            This is a sad verse because Jerusalem was in a sad and horrible state, as America is too. The Lord was saying through his prophet, Ezekiel, that he was searching for someone to intercede for Jerusalem and do what was necessary for Jerusalem’s salvation. The word salvation means the act of saving or protection from harm, risk, loss or destruction. But, when God looked He found no one who would intercede for the nation. This was a bad situation for God and for the nation of Jerusalem. Our country America, who was blessed by God; not because of our people or because of our resources, it is because of our God. But, I am afraid America has for the most part turned its face and its heart from God. We are now reaping what we have sewn. Our only hope is God.

2 Chronicles 7:14 If my people who are called by my name will humble themselves and pray, and seek my face, and turn from their wicked ways, then they will hear from Heaven, and I will forgive their sins and will heal their land.

            These are the conditions that God has said in his word and it’s his promise that if we will do this then he will heal our land. America is only a great country because our God is great. We as a nation, I’m afraid, have told God we no longer need Him to guide us or protect us. But regardless God is a faithful Father who still loves us, and in his love and mercy, He is calling us to be gap people. To stand in the gap to pray for America. To hold America up in prayer. This is the greatest country and please lets pray Gods healing mercy and grace on this great country we call our home.                               Kay

A Love Letter for God Our Father on this Father’s Day

Dear Father God, Jehovah and Daddy,

On this special day on earth we call Father’s Day I would like to remember you my Heavenly Father, who has taught me, guided and instilled wisdom in me, my entire life. 

You have promised me countless times in your word and in my prayer time how much you love. You even went so far to show me how much you love by sending your only son.

Can I ask for anything else? I know I shouldn’t but I always do. As any good Father does, you do what you feel is right and always for my good.

You have always been there for me every day of my life, no matter what I was going through good times or bad times you where there.

This written letter is to thank you for being my Father. I have been blessed by being one of your children. Even though you have countless children I thank you for your undivided attention. And I thank you for always treating me as if I where your only child whenever I talk to you.

Choosing the right Skilled Nursing Facility for yourself or a loved one can be an overwhelming experience that you shouldn’t have to undertake on your own. Do you accept insurance?

Our Skilled unit utilizes skilled insurance benefits such as Medicare, Medicare Alternative Plans, Supplemental Insurance, Medicaid as well as Private Pay.  Our Assisted Living Facility accepts Medicaid and Private Pay

How do I apply to be a client?

To enter one of our facilities a form called an FL-2 completed by A Medical Provider or Medical Facility must be completed and submitted.  Typically this form is completed by the medical social worker at the hospital if entering our Skilled Unit

Do you provide transportation services?

Yes, we provide transportation to medical/dental providers.  These appointments must be initiated by our transportation services coordinator.

Do you offer hospice care?

Yes, Shaire Center collaborates with local Hospice and Palliative organizations to provide services to our clients and their families.

Do you offer laundry Services?

Yes.  All of our facilities offer laundry Services at no additional charge.  We do request all clothing be labeled.
